Prevent and manage chronic health conditions in older Tasmanians

Enhance awareness of the benefits of physical activity

Who is Eligible?

  • Aged 60 and over
  • OR aged 45 and over if Aboriginal or Torres Strait Islander
  • Reside in Tasmania – specifically these regions
  • Are at risk of developing or currently have a chronic health condition
